Within just one week of launching our Proactive Postpartum Call Center, the project saw its first home visit—completed by one of our community health workers, Terra. She visited a family our nurses hadn’t been able to reach by phone. As she approached the door, she could hear the family inside, catching a lighthearted moment: dad asking if mom had ordered food.
Terra introduced herself to the family, congratulated them on their new baby, and explained that the visit was part of a new effort to support families in the early days after delivery. She reassured them that life with a newborn can feel overwhelming, and that she was there to help however she could.
Together, they talked about follow-up care, confirming that mom’s postpartum appointment and baby’s pediatric visit were already on the calendar. She also connected with the couple’s older son, who was proudly adjusting to his new role as big brother. Within minutes, Terra was able to coordinate with a call center nurse to complete a postpartum medical assessment on the spot.
